Healthcare and Insurance

Private hospitals in Thailand's main cities are excellent and far cheaper than in the US, but foreigners aren't covered by the Thai state scheme, so you pay or insure. A private GP visit is about 500-1,500 THB; a knee replacement starts around USD 10,061. The hard part is insurance after 70: Thai-insurer premiums climb steeply and new cover gets scarce, so buy before 70 if you can and choose a plan with guaranteed renewal.

Public or private

Public hospitals are cheap (GP visit 100-500 THB) with skilled doctors but long queues and less English. Private hospitals charge more (500-1,500 THB for a GP, 1,000-3,000 THB for a specialist) and are fast, comfortable and used to foreign patients. Foreign retirees are not covered by Thailand's universal coverage scheme: the answer is no.

Insurance by age

  • 65 to 70: Thai-insurer premiums about 60,000-120,000 THB a year.
  • 70 to 75: 80,000-180,000 THB. Some plans stop taking new applicants around here.
  • 75 to 80: 100,000-250,000 THB, with few insurers still writing new policies.
  • International plans at 70+: USD 9,755-22,822 a year.

The visa insurance rule

Only the O-A visa (and the O-X and LTR at their own levels) require insurance. For the O-A it's at least 3,000,000 THB or USD 100,000 per policy year, including while you extend it in Thailand. The Non-O retirement extension has no insurance requirement.

Medicare and home-country cover

US Original Medicare is usually not covered in Thailand. Some retirees keep Medicare for trips home and buy local cover for Thailand. Check what your home system pays abroad before you cancel anything.

Long-term care

A full-time live-in nurse costs about 35,000-37,500 THB a month and home care 18,000-35,000 THB. Plan who will make decisions for you if you can't: a written health-care directive under Thai law (National Health Act B.E. 2550 s.12) and a named contact at your hospital.

Emergencies

Call 1669 for an ambulance. Carry your insurance card and passport copy, and know which private hospital you want to be taken to.

What are the best options for medical insurance at 71?

Look for plans that still accept new applicants at 71 and guarantee renewal for life, compare a Thai plan with an international one, and declare every condition. Our insurer directory shows entry ages and renewal limits.

Should I insure or self-insure?

Self-insuring only works with a large fund set aside for a serious illness, typically several times the cost of a heart bypass (USD 20,474) plus months of care. Most retirees are better off with at least inpatient cover.

What do hospitals actually cost: Bumrungrad versus a government hospital?

A private room at Bumrungrad runs 6,000-12,000 THB a night; a public hospital costs a small fraction of that but with shared wards and waits. Many retirees use public hospitals for routine care and private ones for emergencies and surgery.

Who will look after me at 80?

Options are home care (18,000-35,000 THB a month), a live-in nurse, assisted living, or a nursing home (20,000-50,000 THB). Decide early, write down your wishes and name a decision-maker.
